Proper control and management of your diabetes can significantly reduce complications of diabetes – including eye disease, kidney disease and nerve damage.
Training for Individuals & Families. Our staff teaches you how to take action to prevent the serious and often life-threatening complications of diabetes, in a series of individual sessions.

Our professional, bilingual staff of nurses and dieticians works closely with referring primary care physicians to help patients manage symptoms and care for diabetes.
We assist with referrals to specialists, including podiatrists, endocrinologists and ophthalmologists.
We speak English and Spanish, and offer translators for other languages.
